Great subject! I must have (i.e. i really dont like to leave the dock
without):

CG-required gear (flares, pfds, etc)
compass
recent chart
tide/current tables if in tidal waters
flashlight
paddle
anchor and rode
assortment of spare rope.
knife w/marlinspike (I got a nice Myerchin knife with marlinspike as a gift
a couple years ago and I don't know how i ever got along without it).

But I usually add:
gps
vhf
depth sounder (hey, some bodies of water and many obstructions just aren't
charted!)
cell phone
tool kit w/
   sail repair tape
   a couple tubes of epoxy stick
   spare lamps for nav lites
   spare spark plugs and wrench
   assorted old blocks and shackles
   siezing wire
   wrenchs
   one of those 4-in-1 screwdrivers
   spare fuses
   some hookup wire and crimp connectors.
   (On the water I have used everything in this kit, or been without it and
needed it).
portapottie
sunscreen
tapes for the stereo
rain gear (i live near seattle!)
spare socks and underwear
bucket or two of margaritas
